ABSTRACT
Neuroleptic drugs, essential in treating psychiatric disorders, often cause hormonal alterations
by disrupting the hypothalamic-pituitary axis through their effects on dopaminergic and
serotonergic pathways. These disturbances can lead to conditions such as hyperprolactinemia,
thyroid dysfunction, metabolic syndrome, and alterations in sex and stress hormones, which
may exacerbate psychiatric symptoms and impair physical health. Effective management
requires a comprehensive understanding of these effects and strategies such as switching to
atypical antipsychotics with lower endocrine impact, dose optimization, or adjunctive therapies
like dopamine agonists or hormonal treatments. However, challenges persist due to
underdiagnosis, limited clinical guidelines, and variability in individual responses. Advancing
research to elucidate the mechanisms behind these alterations and develop targeted
interventions is crucial. A multidisciplinary approach involving psychiatrists, endocrinologists,
and researchers is vital to address these complexities, improve patient outcomes, and minimize
the burden of neuroleptic-associated hormonal side effects.
